Medical Provider Configuration

Overview

Medical providers, both school specific and public providers, can be linked to patient records by adding them as a medical provider in the database.

Medical providers can be searched using the “Find Medical Providers” query on the Health homepage. To view a specific record from the query results, click the “Medical Provider Info” link in the “Medical Provider info” column.

To add a new medical provider, select the ‘Add a Medical Provider’ link under the General section on the Health homepage.

General

  • Prefix: use the drop-down menu to select the appropriate prefix for the medical provider.
  • First Name: enter the first name of the medical provider.
  • Middle Name: enter the middle name of the medical provider.
  • Last Name: enter the last name of the medical provider. This is a required field.
  • Suffix: use the drop-down menu to select the appropriate suffix for the medical provider.
  • Business Phone: enter the business phone of the medical provider.
  • Mobile Phone: enter the mobile phone of the medical provider.
  • Email 1: use this field to specify the preferred email address for the medical provider. This email address will be used for all mass emails and other correspondence.
  • Email 2: use this field to specify a supplemental email address in addition to the primary email address (Email 1).
  • Occupation: use the drop-down menu to select the occupation of the medical provider.
  • Employer: enter the employer information for the medical provider.
  • Job Title: enter the job title of the medical provider.
  • General Medical Notes: use the text box to enter any medical notes regarding the medical provider.

Specialities

The Specialities tab displays a list of the medical provider’s specialities (i.e. pediatrician, etc.) Additional specialities can be added using the input grid. Use the drop-down menu in the ‘Specialty Code’ column to select the specialty. Add any additional comments in the ‘Comment’ column.

Patients

The Patients tab displays a list of all the students that are patients of the medical provider. To view detailed information about each student, select the magnifying glass. Additional student patient records can be added using the input grid.

Histories

The Histories tab displays a list of medical condition histories in which the medical provider has been listed as the provider. To view detailed information about each medical condition history, select the magnifying glass. Additional student medical condition history records can be added using the input grid.

Visits

The Visits tab displays a list of all medical visits in which the medical provider has been listed as the provider of the visit. To view detailed information about each medical visit, select the magnifying glass. Additional medical visits can also be added using the input grid.

Immunizations

The Immunizations tab displays a list of Immunizations administered or scheduled in which the medical provider has been listed as the provider for the immunization. To view detailed information about each student immunization record, select the magnifying glass. Additional immunization records can also be added using the input grid.

Referrals

The Referrals tab displays a list of medical visit records in which the student has been referred to the medical provider. To view detailed information about the referral, select the magnifying glass. Additional referrals can be added using the input grid.

Medications

The Medications tab displays a list of student medication records in which the medical provider has been specified as the provider. To view detailed information about the student medication record, select the magnifying glass. Additional student medication records can be added using the input grid.

Medication Log

If the medication in which the medical provider is listed as the provider has been specified to create log entries, the student’s medication log entries will display on the medical provider’s Medication Log tab. This tab can be used to track whether the medical provide has given the medication or not.

Files

The Files tab displays a list of the medical provider’s uploaded files. To add a new file, select the “Attach Files” link and upload the file using the File Upload service.

Roles

The Roles tab displays a list of all possible roles that can be assigned to the medical provider. When the medical provider is added the role will update automatically to ‘Medical Provider.’ Additional roles can be selected as needed.
