Adding a Behavior Incident

Overview

Behavior incidents are used to identify a student’s behavior, track the incident and outcome of it and notify the necessary people. Note that all sorts of incidents can be tracked with the feature, from violent ones requiring significant action and reporting, down to simple things like missing homework and lack of preparedness that may only need to be tracked.

Add Behavior Incident – Teacher Portal

From the teacher portal homepage, the following steps can be followed to add a new Behavior Incident for a student. On the homepage, navigate to the “Add Student Behavior” button. This will open a detail screen with the following sections to fill in.

Note: Behavior Incidents are not enabled at all schools. If your school would like behavior enabled on the teacher portal, please contact your Veracross Account Manager.

Student

Student

A field to select the student for whom the behavior incident is issued. This field will recognize and display student names that contain the letters you begin to type in the field to help with faster selection.

Class

An optional field to choose a class for which this incident is associated with.  After a student is selected, the class dropdown will display active classes the selected student enrolled in.

Other

After a student is selected, the Name, Grade, Advisor/Homeroom and Parents will be displayed for that student.

Incident

Incident Type

The incident type explains why a behavior event has been entered (e.g. Disrespect toward a teacher, Dress code violation, etc.). This is chosen from a dropdown of preset values configured for your school.

Date

The date for which this incident occurred.

Notes

Details regarding this incident.

All of the Incident fields can be displayed in the notification email template for a behavior incident.

Outcome

Outcome Type

The outcome type explains what the result of the incident is (e.g. Detention, Call home, etc.). This is chosen from a dropdown of preset values configured for your school.

Date

The date for which this outcome is to take place.

Notes

Details regarding this outcome.

All of the Outcome fields can be displayed in the notification email template for a behavior incident.

Add Behavior – Axiom

In Axiom, the following steps can be followed to add a new Behavior Incident for a student. On the school level homepages, navigate to the “Add a Behavior Incident” link. This will open a detail screen with the following sections to fill in.

Student

Status/Follow Up

Status

The status is used to help track the process of the behavior incident.  The status and follow up status can be used to indicate the current state of this event (pending, needs follow-up, closed, etc.).

Date

The date for which the status was set or the follow-up should happen.

Assigned To

Used to identify who is responsible for following up with this behavior incident.

Notification

Template

Identifies which email template should be used for the notification candidates selected. This will most likely always be the public Behavior Notification template.

Notes

Notification notes or comments.

Notification Candidates

On the Notifications tab, Notification Candidates will be listed once a student is selected.
